The dentist will evaluate your teeth, gums, bite (jaw), and previous restorations.
X-rays

Digital images will be taken to better visualize what cannot be seen with the naked eye, such as tooth roots and the jaw.

We will discuss the findings together, answer your questions, and establish the next steps according to your needs and goals.

Medical questionnaire to complete
In order to serve you better, we need to get to know you better. Before your appointment, we ask that you complete a medical questionnaire. This allows us to better understand your health history, allergies, medications, and anything else that may affect your dental care.
The form can be completed at the clinic in paper format. If you are taking multiple medications, we recommend that you bring a list with you.
Please arrive about 10 minutes early to complete it on site.
Insurance and the Canada Dental Care Plan (CDCP)
We make your visit easier by offering direct billing to most private insurers. Simply bring your insurance card with you to your appointment.
Are you eligible for the Canada Dental Plan (CDP)? We are happy to accept this program. Our staff can also help you if you need assistance with your application or to verify your eligibility.
